Utah Industrial Assistance Fund
Economic Opportunity Incentive Application*

Please respond to each question using the same numbering system to correspond with your information (i.e. 1 through 8). Submit the completed Economic Opportunity Incentive Application in a word processing program via e-mail a minimum of 3 weeks prior to the board meeting for the yearly funding round, which generally meets on the 3rd Friday of every month. (See Application Deadlines for specific dates).

1. List your entity's name, mailing and street address, telephone number, fax number, federal tax ID number, and contact person's name.
   
   
2. Describe the Project including critical milestones and benchmarks.
   
   
3. Outline and describe in detail the Return on Investment (ROI) of state funds. Submit an econometric study or suggest a credible method of calculating ROI.
   
   
4. Outline the financial structure for the project and list all funding sources. Please note that state funds are to act in concert with other funding sources.
   
   
5. Identify the problem or economic inhibitor being addressed. Explain why this project is a critical issue. Please refer to the language in the statute to justify use of this fund.
   
   
6. What is the intended outcome should your project be successful in addressing the specific issue identified in Q5. How does this success of this project relate to job creation or other desirable economic indicators?
   
   
7. Describe the project's readiness. Demonstrate how the project is ripe and able to put state funds to work now.
   
   
8. Other information you deem necessary.

Similarly, some information that is not included herein may be requested. GOED staff reserves the right to modify this application for any project. Therefore, please contact staff before submitting an application.
